STATE DLI TARGETS FOR 2014
1. 80% of PBF Health Facilities received every quarterly 2013 payment due to them*
2. 2014 outpatient visits p.c. to PBF HFs increases 15% over baseline*
3. Fully immunized children by PBF HF in 2014 increased by 15% over baseline.*
4. 2014 PBF HF deliveries increased 15% over baseline*
5. Approved 2014 State health budget (i) uses new chart of accounts, (ii) has been published online
6. State published its & LGA 2014 annual DLI Report Card on SPHCDA &/or SMOH websites
7. State released LGA DLI Grants to 80% of participating LGAs within 2 months of Bank

LGA DLI TARGETS FOR 2014
1. LGA approved 2014 health budget uses chart of accounts
2. 50% of participating PBF HF have quarterly supervision visits, produce completed checklist*

  HMIS reports
4. LGA (i) has NOT posted/transferred health staff<LGA Salary Grade 7 to participating HF,
  (ii) has maintained mandatory hiring restrictions unless participating HF staffing below 2012

The Performance Based Financing facility under Part 1.A.1 of the Project has been piloted in at least one LGA in every Participating State by the signing of the PBF Grant Agreement between the SPHCDA and at least one PBF Health Facility, and payments have been made to the PBF Health Facility pursuant to the terms of said agreement. 20% $100,000  
Each Participating State has calculated the baseline for outpatient visits per capita per year in the PBF Health Facilities for the last quarter preceding the Effective Date. 15% $75,000

LGA DLI
The LGA PHC Departments have been provided operational budgets for supervision of the Participating Health Facilities and their staff has been trained in systematic supervision using a standard quality checklist. 20% $10,000  
Each Participating LGA has prepared an annual HMIS report for FY 2011 and HMIS reports for the first two quarters of FY 2012 on the basis of the percentage of Participating Health Facilities reporting. 20% $10,000  
(a) Each Participating LGA has prepared staffing profiles of the Participating Health Facilities in its jurisdiction and has established a baseline for staff below LGA Salary Grade 7, and (b) Each Participating LGA has announced mandatory hiring restrictions for staff below LGA Salary Grade 7 in the Participating Health Facilities, unless the staff in a specific Participating Health Facility is below the baseline established under paragraph (a) above. 20% $10,000 In Ondo State, state-wide staff audits have precluded to the need for a hiring freeze.
Each Participating LGA has adopted guidelines for the procurement of pharmaceutical products by LGAs and health facilities with provisions for purchase of such pharmaceutical products either directly from pharmaceutical manufacturers or suppliers, who possess a manufacturer’s certificate that they are authorized stockiest for the products being procured and they have a valid annual license from the Pharmaceutical Council of Nigeria, and have valid NAFDAC registration. 20% $10,000 In Ondo State, there is an existing procurement contract between the state government and four prime vendors, which ensures that procurement does not occur outside of this arrangement.
